Required Documents
All of the following documents must be submitted before your child can start:
- Birth Certificate
- Social Security Card
- Health Insurance Card
- Updated Immunization Record
- Picture Identification (3 and 4 year olds only)
- 2 Proofs of Address (3 and 4 year olds only)
- Flu shot received between September and December of the current school year
- Child must be present at the time of registration
